How do I fill out form MVU - 26?

The form is rather short with several fields to be completed. The first part of the form comprises information about the vehicle that is transferred, namely:

  • Identification number of the vehicle
  • Title number
  • Prior owner`s name
  • Prior owner`s address, city, state
  • New owner`s name
  • New owner`s address, city, state
  • The date of transference

The second part of the form is the declaration where the new owner and the prior owner should put their signature. If the prior and new owners have  the same last name or live at the same address, only new owner signs the affidavit.
